The "Add a field" palette has 19 types. Use as many fields as a form needs, in any mix.

Typed answers:

  • Single line text: short answers. Supports min and max length and a pattern, with a live counter when you set a max length.
  • Paragraph text: longer messages, with the same length options and the same live counter.
  • Email address: checked in the browser and again on your server. The first Email address field on a form marks the entry as a lead, and it powers the thank you email, Reply-To, ResultsAiX Mailer and WP Sync.
  • Name (first / last): two boxes, saved as one clean name.
  • Phone number and Website / URL: bring up the matching keyboard on phones. An address that is not valid shows "Please enter a valid web address."
  • Number: with optional Min and Max.

Choices:

  • Dropdown, Radio buttons (pick one) and Checkboxes (pick several). Each needs two or more options. All three feed the "Answer breakdown" charts on the AI Insights tab. Radio buttons and Checkboxes are grouped so a screen reader announces the question with its options.
  • Star rating (1-5): one tap, averaged in the same charts. Each star has its own name for screen readers, such as "4 stars".

Date, time and address:

  • Date picker and Time picker use the browser's own pickers.
  • Address: Street address, City, State / Region, Postal code and Country in one block, saved as one line.

Consent and hidden data:

  • GDPR consent: a tick box with your wording, plus optional small print underneath. It is stored as "Consent given". Make it Required and the visitor sees "Please tick the consent box to continue." until they agree.
  • Hidden field: carries a value the visitor never sees. Type a set value, or type a question mark and a name to capture it from the page address. For example ?ref stores the value of ref from a link like yoursite.com/contact/?ref=partner. Hidden values appear on the entry and in the CSV export, and they are never sent to your AI provider when you run AI Insights.

Layout:

  • HTML / content block: your own text, links or simple HTML between fields.
  • Section break: a heading with an optional description, to group a longer form.

File upload:

One file per field, up to 5 MB. Allowed types: JPG, PNG, GIF, WebP, PDF, Word (DOC and DOCX), TXT, CSV and ZIP. The visitor sees "Max 5 MB. Images, PDF, Word, text, CSV or ZIP." under the field.

Uploaded files are private. Only a logged in administrator of your site can open them, through the secure link on the entry and in your notification email. If you click the link while logged out, WordPress asks you to log in first and then opens the file. The visitor's own thank you email shows the file name only, never a download link. Deleting an entry deletes its files too, and so does an erase request from WordPress's Erase Personal Data tool.

Hidden fields, HTML blocks and Section breaks have no Required or Width setting.
